What this trial studies
This study aims to implement a digitized Smart Checklist tailored to patient-provider interactions to enhance the safety of tracheal intubations in pediatric intensive care units (PICUs). The checklist includes prompts based on patient characteristics, displays difficult airway information, and provides high-risk warnings using predictive analytics. Conducted across six hospitals, this stepped-wedge cluster randomized trial will evaluate the checklist's effectiveness in reducing adverse airway outcomes during intubation procedures. The study also seeks to understand the work systems and processes that influence the checklist's clinical impact.
Who should consider this trial
Good fit: Ideal candidates for this study are critically ill pediatric patients who require tracheal intubation in participating PICUs.
Not a fit: Patients undergoing endotracheal tube exchanges will not benefit from this study as it involves a different procedure.

Inclusion Criteria: All patients intubated in the PICU at participating sites Exclusion Criteria: Endotracheal tube exchanges, as this represents a distinct procedure
