What this trial studies
This study aims to explore the metabolic mechanisms linking obstructive sleep apnea (OSA) to impaired glucose metabolism and prediabetes. It will involve patients with OSA and prediabetes undergoing assessments under three different conditions: untreated OSA, treated with continuous positive airway pressure (CPAP), and pharmacologically suppressed lipolysis using Niacin. By analyzing systemic and cellular metabolic pathways, the study seeks to identify potential targets for improving glucose metabolism in these patients. The randomized cross-over design will allow for direct comparisons of the effects of treatment and conditions on metabolic outcomes.
Who should consider this trial
Good fit: Ideal candidates are overweight or obese individuals who have been diagnosed with prediabetes and obstructive sleep apnea.
Not a fit: Patients who are diabetic, have severe hypertension, or are taking medications that could interfere with the study assessments may not benefit from this study.

Inclusion Criteria: * Overweight or obese * Prediabetic * Sleep apnea Exclusion Criteria: * Diabetic * Severe hypertension * Taking medications that can confound assessments * Any history of known bleeding disorders * Any underlying disease likely to limit life span or increase risk of intervention * Currently pregnant, trying to get pregnant or nursing
