This study is testing how airflow problems during asthma attacks are caused by issues in the airways of non-smoking people with moderate to severe asthma, while they are treated with inhaled medications.

What this trial studies

This study aims to investigate the mechanisms behind expiratory airflow limitation in stable, non-smoking asthmatics with moderate to severe persistent airflow obstruction. Participants will receive inhaled corticosteroids and long-acting beta2-agonists while researchers assess whether large or small airways are primarily responsible for airflow limitation. The study will also evaluate the role of inflammation in these airways using exhaled nitric oxide as a marker. Additionally, lung function studies will be compared to pathological analyses from autopsy samples to better understand lung structure and function in asthma.

Who should consider this trial

Good fit: Ideal candidates are non-smoking individuals aged 10-80 with stable, treated asthma and significant airflow obstruction.

Not a fit: Patients who are pregnant or have a significant smoking history may not benefit from this study.

Inclusion Criteria:

* Current non-smoking (\<10 pack yr smoking history)
* Stable, treated asthmatics
* Age 10-80 yr
* post 180ug albuterol by MDI: FEV 1/FVC \< 70% and FEV 1 \<80% predicted

Exclusion Criteria:

* Pregnancy
