This will test whether temporarily clipping the umbilico-uterine trunk plus local epinephrine reduces bleeding during laparoscopic myomectomy in women aged 19–45 with up to four fibroids.

What this trial studies

Women aged 19–45 scheduled for laparoscopic myomectomy for up to four fibroids (largest ≤9 cm) will undergo surgery in which the umbilico-uterine trunk is temporarily clipped and local epinephrine infiltration is used per protocol. Surgical teams will record intraoperative blood loss, transfusion requirement, operative time, and immediate complications. Outcomes will be compared to standard expectations to see if the clipping approach lowers perioperative bleeding and related interventions. Patients with submucosal or dominant pedunculated fibroids, postmenopausal status, suspected malignancy, or major comorbidities are excluded.

Who should consider this trial

Good fit: Women aged 19–45 planning laparoscopic myomectomy with no more than four fibroids and a largest fibroid ≤9 cm who are willing to consent and comply with follow-up are ideal candidates.

Not a fit: Patients who are postmenopausal, have dominant pedunculated subserosal or submucosal fibroids, suspected uterine or adnexal malignancy, or significant medical/psychiatric conditions are unlikely to benefit or be eligible.

Why it matters

Potential benefit: If successful, the technique could reduce operative blood loss and lower the need for transfusions and other bleeding-related interventions.

How similar studies have performed: Related methods such as temporary uterine artery occlusion, tourniquets, and local vasoconstrictors have shown reductions in myomectomy blood loss, but direct evidence specifically for umbilico-uterine trunk clipping is limited.

Eligibility criteria

Inclusion Criteria:

* Age between 19 and 45
* Symptoms of menorrhagia, pelvic pain, or infertility
* Plan to have a laparoscopic myomectomy
* Number of myomas ≤ 4, with the largest myoma measuring ≤ 9 cm

Exclusion Criteria:

* Postmenopausal status
* Dominant pedunculated subserosal FIGO classification (6-7)
* Submucosal type of myoma FIGO classification (0-2)
* Suspicion of malignant uterine or adnexal diseases
* Major medical comorbidities or psychiatric illnesses that could affect follow-up and/or compliance
* Refusal to participate or provide consent to the procedures.
