This project will test whether a Turkish sleep questionnaire combined with actigraphy can accurately find sleep problems in children and adolescents with complex chronic conditions during and after a PICU stay.

What this trial studies
This multi-phase, prospective observational project first translates and culturally adapts the Sleep Screening Tool for Children and Adolescents with Complex Chronic Conditions (SCAC) into Turkish using forward/backward translation, expert review, caregiver interviews, and a pilot. Psychometric testing will include exploratory factor analysis, internal consistency (Cronbach's alpha), and test-retest reliability. In a second phase, children admitted to participating PICUs will have continuous actigraphy and bedside light/noise monitoring during hospitalization, with follow-up monitoring at home to compare sleep patterns. The study will also quantify types of sleep problems and relate them to diagnosis, age, sex, medications, devices, and environmental factors within a family-centered framework.
Who should consider this trial
Good fit: Children aged 1 month to 18 years with at least one complex chronic condition who are admitted to the participating PICU and have parents who are literate in Turkish and willing to participate.
Not a fit: Children whose caregivers cannot read Turkish, children not admitted to the participating PICUs, and children without complex chronic conditions are unlikely to benefit directly from this validation effort.
Why it matters
Potential benefit: If successful, clinicians and families in Türkiye would have a validated Turkish screening tool to help spot sleep problems early and guide care for children with complex chronic conditions.
How similar studies have performed: Related pediatric sleep screening tools and the original SCAC have been used and validated elsewhere, but creating and validating a Turkish SCAC for children with complex chronic conditions is novel.

Inclusion Criteria: Age 1 month to 18 years At least one complex chronic condition (per 2014 CCC classification) Admission to a pediatric intensive care unit (PICU) Parents willing to participate Parents literate in Turkish Exclusion Criteria: Parents with insufficient Turkish language proficiency to complete study materials
