This will try a focused physical therapy program to strengthen the soleus muscle and correct anterior pelvic tilt to improve crouch gait in walking children with cerebral palsy aged 4 to 10.

What this trial studies

This interventional program enrolls ambulatory children aged 4–10 with cerebral palsy who present with crouch gait, anterior pelvic tilt, and soleus weakness confirmed by EMG. Participants undergo a structured physical therapy program emphasizing soleus strengthening and exercises to improve pelvic alignment. Muscle function and gait changes are monitored with EMG and clinical gait observation over the treatment period. Children with prior selective dorsal rhizotomy, posterior pelvic tilt, non-ambulatory status, or BMI/age outside the protocol ranges are excluded.

Who should consider this trial

Good fit: Ideal candidates are ambulatory children with cerebral palsy aged 4–10 who have crouch gait, measurable anterior pelvic tilt, and soleus weakness on EMG, with BMI between 18.5 and 21 and no history of selective dorsal rhizotomy.

Not a fit: Children who are non-ambulatory, have posterior pelvic tilt, a history of selective dorsal rhizotomy, are outside the 4–10 age range, or whose BMI is outside 18.5–21 are unlikely to benefit.

Why it matters

Potential benefit: If successful, the program could improve walking posture and function and reduce the need for more invasive interventions in children with crouch gait.

How similar studies have performed: Prior research has typically targeted hamstring stretching and quadriceps strengthening with only partial correction of crouch gait, and targeting the soleus and pelvic tilt is a relatively novel approach with limited published evidence.

Inclusion Criteria:

patient with cp have a crouch gait. ages from 4 to 10 years . have a anterior pelvic tilt . muscle weakness (Soleus) on EMG.

* BMI from 18.5 to 21..
* didn't submit to Selective Dorsal Rhizotomy surgery.

Exclusion Criteria:

have a posterior pelvic tilt.

-above 10 or below 4 years. not able to walk. . submitted to SDR surgery. BMI above 21 or below 18.5.
