What this trial studies
This retrospective observational cohort uses routinely collected electronic health records from five Brazilian municipalities participating in the epHealth Primary Care Solutions platform to characterize treatment patterns for hypertension. Adults (≥18) will be identified by recorded hypertension diagnoses or by at least two consecutive blood pressure measurements ≥140/90 mmHg within 30 days and followed for a minimum of six months. Primary outcomes focus on diagnostic and therapeutic clinical inertia, including timing of treatment changes and alignment with national guideline recommendations rather than testing an intervention. The design leverages existing EMR data to capture real-world practice across diverse socio-demographic and healthcare settings within Brazil's public primary care system.
Who should consider this trial
Good fit: Adults aged 18 or older with a recorded hypertension diagnosis or at least two consecutive high blood pressure readings in the participating municipalities' EMRs and with at least six months of continuous follow-up in the same municipality are the ideal candidates for inclusion.
Not a fit: People whose records are incomplete, not traceable, or who receive care outside the participating municipalities are excluded and unlikely to benefit directly from this analysis.
Why it matters
Potential benefit: If successful, the findings could reveal gaps and delays in hypertension care and guide changes to improve blood pressure control across Brazil's public primary care system.

Inclusion Criteria: * Age ≥18 years at the time of Hypertension diagnosis * Hypertension diagnosis as retrieved from registered ICD codes or CIAP Codes (primary care codes) in the EMR , OR At least two blood pressure measurements ≥140/90 mmHg during the study period. The blood measurements should be consecutive and obtained within a 30 days period. * Continuous follow-up in the same municipality for at least 6 months after index measurement. * Availability of structured EMR data including clinical encounters, diagnoses, and prescriptions Exclusion Criteria: \- Patients with incomplete data or not traceable in the registration systems
