What this trial studies

This clinical trial focuses on patients who have experienced failed surgical treatment for carpal tunnel syndrome (CTS). It aims to evaluate the effectiveness of a dorsal ulnar artery perforator adipofascial flap as a novel intervention for those with persistent or recurrent symptoms after initial surgery. The study will include patients aged 18-60 who exhibit a positive Tinel sign and have had their initial surgery between 3 months and 2 years prior. Exclusion criteria include patients with cervical disc issues or other significant medical conditions that could affect nerve function.

Inclusion Criteria:

* Age 18-60 years
* Positive tinel sign
* Operative interval more than 3 months \& less than 2 years

Exclusion Criteria:

* Cervical disc
* Associated general disease (Rheumatoid arthritis, DM ,psychological problems \& peripheral neuropathy)
