What this trial studies
This study aims to address the challenges faced by young adults transitioning from pediatric to adult care for type 1 diabetes (T1D). It tests a novel transdisciplinary model of care designed to improve health outcomes and psychosocial well-being during this critical transition period. All study procedures, including enrollment and evaluations, are conducted remotely, allowing for nationwide participation. The focus is on assessing the feasibility, acceptability, and initial efficacy of this new care model.
Who should consider this trial
Good fit: Ideal candidates are young adults diagnosed with type 1 diabetes for at least six months who are nearing the end of their pediatric care.
Not a fit: Patients with developmental disabilities that limit independent living or other systemic chronic illnesses may not benefit from this study.
Why it matters
Potential benefit: If successful, this approach could significantly improve glycemic control and overall well-being for young adults with type 1 diabetes during their transition to adult care.
How similar studies have performed: While there is limited evidence-based intervention for this specific transition, similar approaches in chronic disease management have shown promise.
