This trial will test whether TransAction—individual risk-reduction sessions, skill-building and group support, plus social events—helps transgender women in Ho Chi Minh City move forward in HIV prevention and care compared with social events alone.

What this trial studies

Eighty transgender women will be randomly assigned to receive the full TransAction package (individual risk-reduction sessions, skill-building and open group support sessions, and social events) or to a control condition invited to social events only. Outcomes including movement along the HIV prevention and care continua, sexual risk behavior, substance use, self-efficacy in seeking services, social support, stigma, well-being, and quality of life will be measured at baseline, 3 months, and 6 months. The study will also document implementation outcomes such as fidelity, adaptation, feasibility, acceptability, and sustainability using implementation records, investigator evaluation, and surveys and focus groups with peer facilitators and participants. The trial is led by UCLA in collaboration with local and international partners and is conducted at the University of Medicine and Pharmacy in Ho Chi Minh City.

Who should consider this trial

Good fit: Ideal candidates are transgender women (male at birth, currently self-identified as transgender women) aged 16 or older who live in or near Ho Chi Minh City, have no plans to move in the next six months, and can give informed consent.

Not a fit: People unlikely to benefit include those who are not transgender women, who do not live in the Ho Chi Minh City area or cannot attend in-person sessions, who previously participated in adaptation activities with the team, or who are unable to provide informed consent.

Why it matters

Potential benefit: If successful, TransAction could increase engagement in HIV prevention and care, reduce risk behaviors, and improve social support and quality of life for transgender women in the area.

How similar studies have performed: Peer-led, skills-building and social support interventions for transgender women and other key populations have shown promise in improving prevention and care engagement, but adaptation and implementation evidence specifically in Vietnam is limited.

Inclusion Criteria:

* 16 years of age or older
* being male at birth and currently self-identified as a TW
* currently living in Ho Chi Minh City adjacent areas and having no plans to move out of the areas in the next 6 months - have the cognitive capacity to participate in study activities as judged by the study recruiter.

Exclusion Criteria:

* have been involved in intervention adaptation activities previously with the study team
* inability to give informed consent/assent
