What this trial studies
This project aims to implement and evaluate a shared decision-making tool called DDInteract, designed to assist healthcare providers and patients in managing drug interactions while on oral anticoagulants. The tool will be integrated into electronic health records (EHR) at three university-based healthcare systems, allowing for the retrieval of patient-specific risk factors and the calculation of potential harm from drug interactions. By enabling dynamic exploration of treatment scenarios, DDInteract seeks to optimize patient care and minimize risks associated with anticoagulant therapy. The study will also assess the effectiveness of DDInteract in real-world settings and identify barriers to its adoption.
Who should consider this trial
Good fit: Ideal candidates for this study are individuals aged 21 and older who are prescribed multiple oral anticoagulants.
Not a fit: Patients who are unable to speak English or Spanish will not benefit from this study due to language barriers.

Inclusion Criteria: * Be at least 21 years of age or older * Receive more than one prescription of an oral anticoagulant (i.e. warfarin, apixaban, edoxaban, dabigatran, rivaroxaban) Exclusion Criteria: * Individuals not able to speak English or Spanish
