This will test whether a fluoride varnish that contains titania nanoparticles reduces dental plaque more than regular fluoride varnish in people aged 16–30 who are starting fixed orthodontic braces.

What this trial studies

This interventional trial compares a novel fluoride varnish loaded with titania nanoparticles synthesized from Moringa oleifera leaf extract to a conventional fluoride varnish in patients receiving multibracket fixed orthodontic treatment. Participants aged 16–30 with no prior orthodontic treatment or active periodontal disease are randomized to receive one of the two varnishes applied during their orthodontic care. The study measures antiplaque and antibiofilm effects over time using clinical plaque indices and likely microbiological assays to track changes in biofilm. Patients on antibiotics, with craniofacial abnormalities, or with mixed dentition are excluded.

Who should consider this trial

Good fit: Ideal candidates are people aged 16–30 who are planning to start fixed orthodontic treatment, have no active periodontal disease, no prior orthodontic treatment, and are not taking antibiotics.

Not a fit: Patients with active periodontal disease, currently on antibiotic therapy, with craniofacial abnormalities, mixed dentition, or who cannot attend the Islamabad site are unlikely to benefit or be eligible.

Why it matters

Potential benefit: If successful, the nanoparticle-enhanced varnish could better prevent plaque buildup around braces and reduce the risk of cavities and gum problems during orthodontic treatment.

How similar studies have performed: Laboratory and small clinical studies of titania and other nanoparticle-enhanced dental materials have shown antibacterial effects, but direct clinical comparisons in fixed orthodontic patients remain limited and relatively novel.

Inclusion criteria:

* Patients planned for fixed orthodontic treatment.
* Patients within age group 16-30 years.
* Patient with no history of previous orthodontic treatment.
* Patients with no active periodontal diseases.

Exclusion criteria:

* Patient undergoing any antibiotic therapy.
* Patients with any craniofacial abnormality.
* Patient with mixed dentition
