What this trial studies
The cohort enrolled nearly all permanent residents aged 18 and older in the Tianshan region beginning in 2016 and collected baseline data on demographics, medical and family history, disability, living environment, and blood biochemical tests. Follow-up is event-driven and comes from chronic disease management programs, annual health examinations, electronic health records from 163 hospitals plus 63 Traditional Chinese Medicine hospitals, and local death registries with ICD-10 coding. The dataset links health assessments, lab results, hospital inpatient records, mortality data, and environmental and statistical datasets to track disease occurrence and outcomes across the population. The program also provides structured health education and community-based prevention activities to support behavior change and reduce modifiable risk factors.
Who should consider this trial
Good fit: Ideal participants are permanent residents of the region aged 18 or older who have lived locally for at least five years, are enrolled in local social security medical insurance, can communicate normally, and can give written informed consent.
Not a fit: Temporary residents, people who cannot complete follow-up due to severe illness or disability, and those unwilling to participate or provide consent are unlikely to benefit from the program.

Inclusion Criteria: 1. Residents \>=18y 2. Permanent residents or individuals who have lived in the region for more than 5 years and are enrolled in local social security medical insurance 3. Residents without severe physical disability and capable of normal communication 4. Residents whose diseases and deaths are monitored and managed by the local health authorities 5. Residents who voluntarily participate and provide written informed consent Exclusion Criteria: 1. Temporary residents or floating population 2. Residents with severe health conditions that prevent participation in the study 3. Residents who are unwilling to undergo follow-up assessments for the study
