What this trial studies
The Brazilian Heart Insufficiency with Telemedicine (BRAHIT) initiative aims to improve care for heart failure patients in Rio de Janeiro by implementing a telemedicine model that connects primary care with hospital cardiologists. This approach seeks to address the challenges of healthcare access and resource scarcity in Brazil, particularly for frail elderly patients. Inclusion Criteria: * Discharge from Hospital with one of the following ICD-10 diagnoses: (DI-11.0,-13.0, 42.0, 42.6, 42.9, 50.0, 50.1, 50.9) and follow-up from INC Hospital Exclusion Criteria: * Patients not willing to participate
