This project will try telemedicine plus a specially trained advanced practice nurse to coordinate personalized care for people with Parkinson's in the RhineMain+ region.

What this trial studies

The program implements an interdisciplinary, cross-sector telemedical network in the existing ParkinsonNetz RheinMain+ where advanced practice nurses (APNs) perform in-depth assessments and coordinate individualized treatment plans. APNs use audiovisual telemedicine to monitor, control and adjust care paths and are empowered to initiate delegated treatment interventions in line with clinical guidelines. The model includes training for other professional groups and involvement of relatives to ensure continuity across outpatient and inpatient settings. Services are integrated with standard treatment pathways and delivered from regional centers in Mainz and Homburg.

Who should consider this trial

Good fit: Adults with primary or atypical Parkinson's disease who live in the Hesse or Rhineland-Palatinate care regions and are able to give informed consent are the intended participants.

Not a fit: Patients with severe dementia, uncontrolled psychiatric disorders, active substance addiction, pregnancy or breastfeeding, insufficient language or literacy skills, or who live outside the defined care regions are unlikely to benefit from this program.

Why it matters

Potential benefit: If successful, patients could receive more guideline-based, better coordinated care that improves symptom management and smooths transitions across care settings.

How similar studies have performed: Telemedicine and nurse-coordinated care have shown benefits in Parkinson's in prior studies, but the specific APN-led, cross-sector telemedical network approach is relatively novel.

Eligibility criteria

Inclusion Criteria:

* Diagnosis of primary Parkinson's syndrome (G20.-) or atypical Parkinson's syndrome (G23.-, according to the current diagnostic criteria of the Movement Disorder Society, including the criteria of at least bradykinesia and rigidity or resting tremor must be present)
* In the care regions of Hesse and Rhineland-Palatinate
* Ability to give consent present

Exclusion Criteria:

\- Severe dementia

* Severe depression
* Psychoses or other psychiatric or medical comorbidities that could affect the smooth implementation of the study protocol (e.g.

tumor disease with limited life expectancy, need for dialysis, etc.)

* Drug or alcohol addiction
* Women who are pregnant or breastfeeding
* Simultaneous participation in another interventional treatment study
* Illiteracy or insufficient language skills
