What this trial studies
The Get-HEALTH-I initiative is a prospective interventional program aimed at assessing the effectiveness and cost-efficiency of a geriatric telemanagement model for elderly patients with multiple health issues who have recently been discharged from hospitals to nursing homes. This model seeks to integrate geriatric specialists with nursing home staff and general practitioners to enhance patient care. The study will evaluate various outcomes, including hospitalization rates, geriatric syndromes, medication management, and overall satisfaction with the intervention in selected nursing homes in the Marche Region of Italy.
Who should consider this trial
Good fit: Ideal candidates for this study are nursing home residents who have been discharged from the hospital within the past week.
Not a fit: Patients admitted to nursing homes for short-term stays may not benefit from this telemanagement program.
Why it matters
Potential benefit: If successful, this program could significantly improve health outcomes and reduce hospital readmissions for elderly nursing home residents.
How similar studies have performed: Other studies have shown promising results with telemanagement approaches for elderly patients, indicating potential for success in this novel application.
