This project tests whether a coordinated lifestyle program of nutrition, exercise, and self-monitoring can help adults at risk for high blood pressure—particularly African Americans in the Nashville area—lower and control their hypertension.

What this trial studies
The intervention delivers a lifestyle immersion program combining AHA Life's Simple 7 principles, the DASH eating plan, physical fitness coaching, and blood-pressure self-monitoring, led by certified experts. Participants receive motivation and skill-development support to help adopt and sustain dietary, activity, tobacco-cessation, alcohol-reduction, sleep, and self-management changes. The program is targeted to adults aged 30–80 who live in Davidson and surrounding counties and are already diagnosed with or at increased risk for hypertension. Activities and follow-up occur through Meharry Medical College with a 12-month commitment from participants.
Who should consider this trial
Good fit: Adults aged 30–80 who live in Davidson or surrounding Tennessee counties, can consent, plan to remain in the area for 12 months, and are diagnosed with or at increased risk for hypertension are the intended participants.
Not a fit: People who live outside the specified counties, are younger than 30 or older than 80, or are not at increased risk for hypertension would not be eligible and are unlikely to benefit from this program.
Why it matters
Potential benefit: If successful, the program could help participants lower blood pressure, improve cardiovascular risk factors, and reduce the future burden of heart disease, diabetes, kidney disease, and obesity through sustained lifestyle change.
How similar studies have performed: Previous trials using DASH and AHA Life's Simple 7-style lifestyle interventions have produced blood-pressure reductions, although long-term adherence and benefits in African American populations have been variable.

Inclusion Criteria: * Reside in Davidson and surrounding counties of Tennessee. * At least 30 years old. * At increased risk for developing hypertension (already diagnosed or a strong family history). * Be able to consent. * Not planning to move from this study area in 12 months after enrollment. Exclusion Criteria: * Does not reside in Davidson and surrounding counties. * Less than 30 years old, or older than 80 years. * Not at assessed increased risk for hypertension.
