What this trial studies
This clinical trial investigates the effectiveness of posterior column osteotomies (PCO) in correcting spinal curvature in adolescents with idiopathic scoliosis. It is a randomized, multicenter trial involving sixty adolescents who will be assigned to either a PCO group or a control group receiving standard treatment without PCO. The study aims to compare the outcomes in terms of coronal and sagittal plane curve correction following posterior spinal fusion. The hypothesis is that the PCO group will show superior correction compared to the control group.

Good fit: Ideal candidates are adolescents aged 10 to 21 years with a Cobb angle of 45 degrees or more due to adolescent idiopathic scoliosis.
Not a fit: Patients with conditions such as diabetes mellitus, bleeding disorders, or those requiring anteroposterior surgery will not benefit from this study.

Inclusion Criteria: * Adolescent idiopathic scoliosis (Lenke 1 to 4) * Age at surgery between 10 and 21 years * Cobb angle of 45 degrees or more * Posterior spinal fusion Exclusion Criteria: * Anteroposterior surgery * Vertebral column resection * Smoking * Diabetes mellitus * Bleeding disorder
