What this trial studies
This study evaluates a culturally responsive care navigator intervention called AURORA, aimed at younger Latinx individuals recently diagnosed with type 2 diabetes. The intervention includes three telemedicine visits where a care navigator assesses the patient's familiarity with diabetes care, identifies self-management needs, provides motivational support, and offers skills training. The study also aims to document the process of hiring a care navigator and refine the AURORA strategy for broader implementation within Kaiser Permanente. The ultimate goal is to enhance diabetes management and support for this specific population.
Who should consider this trial
Good fit: Ideal candidates are Kaiser Permanente members aged 21 to 45 who have been newly diagnosed with type 2 diabetes within the last 1-8 months.
Not a fit: Patients with type 1 diabetes or those who are pregnant will not benefit from this study.
Why it matters
Potential benefit: If successful, this program could significantly improve diabetes management and health outcomes for younger Latinx adults.
