This will try daily stretching and sacroiliac mobilization exercises to see if they reduce muscle and joint pain and improve quality of life in young adults starting isotretinoin.

What this trial studies
Adults aged 18–35 who are beginning isotretinoin and do not have a regular exercise habit will be enrolled and asked to follow a program of stretching and sacroiliac joint mobilization exercises. The exercise program will be delivered at the study site with scheduled follow-ups to record musculoskeletal pain and quality-of-life outcomes. Pain severity, functional measures, and quality-of-life scores will be tracked during treatment and after isotretinoin cessation to observe changes over time. People with diagnosed rheumatologic, neurologic, or psychiatric disorders or physical disabilities that prevent exercise will be excluded.
Who should consider this trial
Good fit: Ideal candidates are adults 18–35 who are starting isotretinoin for the first time, do not have a regular exercise routine, and have no diagnosed rheumatologic, neurologic, psychiatric conditions, or exercise-preventing disabilities.
Not a fit: Patients with existing rheumatologic, neurologic, or psychiatric diagnoses or those with physical disabilities that prevent exercise are unlikely to benefit from this intervention.
Why it matters
Potential benefit: If successful, the program could reduce isotretinoin-related musculoskeletal pain and speed recovery, improving patients' day-to-day function and quality of life.
How similar studies have performed: This approach is novel for isotretinoin-related sacroiliitis—there are no published trials specifically prescribing exercise for this group, though exercise benefits musculoskeletal pain in other conditions.

Inclusion Criteria: * The participant is between the ages of 18-35, * Not having a regular exercise habit, * Starting Isotretinoin treatment for the first time. Exclusion Criteria: * Presence of any diagnosed rheumatologic disease, * The presence of any diagnosed neurological disease, * Having any diagnosed psychiatric illness, * Having a physical disability that prevents them from exercising for any reason.
