Steady Together Fall Prevention Program in the Home
This program will try an occupational therapy–led home visit and online tools to reduce fall risk and increase safety confidence for people with Parkinson's who are InMotion members.

What this trial studies
Using the HfLD platform, occupational therapists work with participants and care partners to identify up to five high-risk rooms and observe typical daily activities during a two-hour in-home session after a 30-minute online intake questionnaire. For each room the therapist records a Personal Safety Rating (0–10) and an Accessibility Rating (0–10), and the HfLD application converts scaled scores to percentages to guide recommendations. The OT provides tailored recommendations for assistive equipment, performance strategies, home modifications, and helpful resources to improve accessibility. Outcomes include changes in self-reported home safety confidence and objective accessibility scores before and after the intervention.
Who should consider this trial
Good fit: Adults (18+) with Parkinson's who are members of InMotion, able to provide informed consent and who have completed the MoCA and have concerns about falls or home accessibility are ideal candidates.
Not a fit: People who cannot make their own medical decisions, live outside the program's service area, are institutionalized, or have severe cognitive or mobility limitations that prevent participation in home assessment may not benefit.
