This will test whether using a standardized method for giving twice-daily subcutaneous LMWH injections reduces bruising and pain for people with cardiovascular disease in cardiac care units.

What this trial studies
This interventional study compares a standardized protocol for subcutaneous low-molecular-weight heparin (LMWH) injections with the current active control procedure in cardiac care units. Nurses will apply the standardized injection technique to patients prescribed twice-daily LMWH and outcomes such as injection-site bruising and pain will be recorded. The protocol aims to reduce administration-related complications by improving consistency of technique across clinicians. Participants are Chinese adults in Hong Kong cardiac units who can provide consent.
Who should consider this trial
Good fit: Chinese adults in cardiac care units in Hong Kong with cardiovascular disease who are prescribed twice-daily subcutaneous LMWH and can consent are the ideal candidates.
Not a fit: Patients who are unconscious, cognitively impaired, refuse to participate, or who are not receiving twice-daily subcutaneous LMWH are unlikely to benefit from this protocol.
Why it matters
Potential benefit: If successful, the protocol could reduce injection-site bruising and pain and improve patient comfort and safety during LMWH treatment.
How similar studies have performed: Protocol-based approaches to subcutaneous injections have reduced injection-site complications in other settings, but specifically standardized LMWH protocols in cardiac units have been limited.

Inclusion Criteria: * patients who are in cardiac care units suffering from cardiovascular diseases prescribed with LMWH injections two times per day * Chinese Exclusion Criteria: * cognitively impaired * unconscious * refuse to participate
