SPICE: Community-based pathway to screen and prevent declines in older adults' physical and mental abilities in Singapore
Implementation of Decade of Healthy Ageing Action Plan to Screen and Prevent Decline in Intrinsic Capacity in Elders (SPICE) Through Multisectoral Collaboration in Singapore
This program tries a digital, community screening and targeted support pathway to prevent declines in physical and mental abilities for adults aged 60 and older in Singapore.

What this trial studies
The SPICE pathway uses WHO ICOPE digital screening tools in community venues to identify early declines across mobility, cognition, nutrition, psychological, and sensory domains. Participants who screen positive receive structured in-depth assessments, traffic-light risk stratification, and personalized care plans linking community interventions or medical referral as needed. The intervention package emphasizes multidomain supports such as exercise, nutrition, cognitive stimulation, and social engagement delivered through a hub-and-spoke model anchored by regional health services. Longitudinal follow-up will track feasibility, uptake, functional trajectories, adherence, and cost-effectiveness of the implementation.
Who should consider this trial
Good fit: Adults aged 60 years or older who are robust or pre-frail, able to give consent and follow instructions, are the intended participants.
Not a fit: People who are already frail or have a terminal illness were excluded and are unlikely to benefit from this prevention-focused pathway.
Why it matters
Potential benefit: If successful, this could help older adults maintain function and delay progression to frailty by detecting early decline and providing targeted support.
How similar studies have performed: Similar ICOPE-based and multidomain prevention programs have shown promising results in pilot implementations, though large-scale long-term evidence is still limited.
