Screening for Ischemic Stroke in the Posterior Circulation Using MRI
Medical Imaging Screening for Posterior-circulation Ischemic Stroke:a Multicentre,Imaging-based,Prospective Cohort Study
Tianjin Huanhu Hospital · NCT06094569
This study is testing if using MRI before surgery can help doctors choose the right patients with certain types of strokes to see if they will recover better after a specific treatment.

What this trial studies
This observational study evaluates the effectiveness of preoperative magnetic resonance imaging (MRI) in selecting patients with posterior circulation ischemic stroke who are likely to benefit from mechanical thrombectomy. It focuses on the pons-midbrain-medulla index (PMMI) to assess the degree of ischemia and correlate it with clinical outcomes 90 days post-surgery. By analyzing MRI images, the study aims to improve patient selection for endovascular thrombectomy, potentially leading to better recovery outcomes. The study is conducted as a multicenter, prospective cohort analysis involving several hospitals in Tianjin.
Who should consider this trial
Good fit: Ideal candidates include patients admitted with acute ischemic stroke confirmed by MRI, specifically those with vertebrobasilar artery occlusion.
Not a fit: Patients who cannot undergo MRI due to contraindications will not benefit from this study.
Why it matters
Potential benefit: If successful, this study could enhance patient selection for mechanical thrombectomy, leading to improved clinical outcomes for those with posterior circulation ischemic strokes.
How similar studies have performed: Previous studies have indicated that MRI can lead to better clinical outcomes compared to CT in similar patient populations, suggesting a promising approach.
Eligibility criteria

Inclusion Criteria: 1. Patient admitted from Jun. 1st 2019 to Jan. 1st 2029; 2. Patients with acute ischemic stroke recently; 3. Symptoms persisted at admission, and MRI (including DWI) confirmed acute ischemic vertebrobasilar artery occlusion in the posterior circulation:(single right or left intracranial branch of vertebral artery occlusion ; single basilar occlusion ; both two vertebral arterys and basilar artery); Exclusion Criteria: 1.Patiens can not meet the MRI examination conditions(with absolute and relative contraindications).
