We will test a nursing program based on the salutogenic model to see if it improves sense of coherence and helps people aged 60–70 with diabetes or high blood pressure living in Turkish nursing homes stay healthier and more independent.

What this trial studies

This interventional project delivers a salutogenic model–based healthy aging program through public health nursing to residents of Turkish Ministry–affiliated nursing homes in Ankara. It enrolls people aged 60–70 who have had diabetes or hypertension for at least three years and who score 24 or higher on the MMSE, while excluding those with major cognitive, sensory, or functional impairments. Trained nurses provide structured activities and guidance aimed at strengthening personal and social resources and promoting healthy behaviors that support a stronger sense of coherence. Outcomes focus on changes in sense of coherence and participants' ability to maintain independent daily functioning.

Who should consider this trial

Good fit: Ideal candidates are residents of Ankara nursing homes aged 60–70 with diabetes or hypertension for at least three years, with MMSE scores of 24 or higher and the ability to perform daily activities independently.

Not a fit: People with known cognitive impairment, severe hearing or vision loss, inability to perform daily activities independently, recent weight loss greater than 3 kg, incontinence, or activity intolerance are unlikely to be eligible or to benefit from the program.

Inclusion Criteria:

* Age between 60 and 70 years.

Residing in nursing homes affiliated with the Turkish Ministry of Family and Social Services in Ankara.

A minimum score of 24 on the Mini-Mental State Examination (MMSE).

Diagnosed with Diabetes Mellitus (DM) or Hypertension (HT) for at least the past 3 years.

Exclusion Criteria:

* Known cognitive impairment or difficulty in perception.

Severe hearing or vision impairment.

Inability to participate in the study due to activity intolerance.

Inability to perform daily living activities independently.

Weight loss greater than 3 kg in the past month.

Presence of incontinence.
