This project will try two approaches—supporting doctors and supporting patients—to increase genomic (NGS) testing for men with metastatic prostate cancer at Johns Hopkins.

What this trial studies
National guidelines recommend next-generation sequencing (NGS) for metastatic prostate cancer, but testing rates are low and around 50–60% at Johns Hopkins. This initiative uses a multidisciplinary quality-improvement approach that targets two barriers: provider-level and patient-level factors. Interventions include education and workflow changes for clinicians and tailored patient-facing supports, with measurement of NGS ordering rates before and after the interventions. The team aims to reduce disparities by focusing on groups less likely to be tested, such as older and unmarried patients.
Who should consider this trial
Good fit: Men with metastatic prostate cancer who are new patients in Johns Hopkins Medical Oncology between January 1, 2025 and December 31, 2026 and who have not yet had NGS testing are eligible.
Not a fit: Patients who have already had NGS testing performed or who do not receive care at Johns Hopkins are unlikely to benefit from this initiative.
Why it matters
Potential benefit: If successful, more patients would receive guideline-recommended genomic testing, improving treatment selection and access to clinical trials.
How similar studies have performed: Related provider- and patient-focused interventions have increased uptake of guideline testing in other settings, but this combined approach specifically for NGS in metastatic prostate cancer is relatively untested at Johns Hopkins.

Inclusion Criteria: * men diagnosed with metastatic prostate cancer * seen by Johns Hopkins Medical Oncology as a new patient visit between January 1, 2025 and December 31, 2026 Exclusion Criteria: \- Patients who have already had NGS testing performed.
