This project will see how placental anatomy relates to outcomes for families with monochorionic twins by analyzing placentas after birth at a Birmingham NHS site.

What this trial studies

This prospective, single-site observational project will recruit people carrying monochorionic twin pregnancies during the antenatal period and obtain consent for placental analysis after delivery. All placentas transferred to the pathology department will be examined macroscopically and key anatomical features will be recorded. These placental findings will be compared with antenatal complications and perinatal outcomes for each twin pair. The goal is to build a comprehensive database to better understand how placental architecture may inform antenatal care.

Who should consider this trial

Good fit: Ideal candidates are people pregnant with monochorionic twins who consent antenatally and deliver at the site such that the placenta can be transferred to the pathology department for analysis.

Not a fit: Patients with singleton, dichorionic, higher-order pregnancies, major fetal anomalies or aneuploidy, or those who do not deliver at the recruiting site are not eligible and would not benefit from this project.

Why it matters

Potential benefit: If successful, the findings could help clinicians tailor antenatal surveillance based on placental anatomy and potentially improve perinatal outcomes for monochorionic twins.

Inclusion Criteria:

• All consecutive placentas of monochorionic twin pregnancies whereby the placenta is transferred to the pathology department for analysis.

Exclusion Criteria:

* Singleton, dichorionic and higher order pregnancies (e.g. triplet).
* Fetus with known major congenital anomalies or aneuploidy
