The Impact of MTM Model on the Efficacy and Safety of Anticoagulant Therapy in Postoperative Colorectal Cancer Patients
This will try a pharmacist-led Medication Therapy Management (MTM) program to reduce blood clots and bleeding in people who had colorectal cancer surgery and are taking anticoagulants.

What this trial studies
This interventional program integrates multifaceted pharmacist interventions at admission, during hospitalization, and after discharge to manage anticoagulant therapy in postoperative colorectal cancer patients with venous thromboembolism. The MTM model includes medication reconciliation, monitoring, individualized dosing and follow-up counseling to reduce inappropriate prescribing, premature treatment discontinuation, and adverse events. Participants are patients with histologically confirmed colorectal cancer who developed symptomatic or incidental VTE and are on active anticoagulant therapy for at least three months. The study compares outcomes of MTM-managed care against usual care to measure recurrent VTE, bleeding events, and overall medication appropriateness.
Who should consider this trial
Good fit: Ideal candidates are postoperative colorectal cancer patients with symptomatic or incidental VTE who are currently receiving active anticoagulant therapy and have been treated for at least three months.
Not a fit: Patients who are not on active anticoagulant therapy, who have already completed anticoagulation, or who cannot attend hospital-based follow-up are unlikely to benefit from this program.
Why it matters
Potential benefit: If successful, the MTM program could lower recurrent clots and bleeding events and improve the appropriateness and continuity of anticoagulant treatment for postoperative colorectal cancer patients.
How similar studies have performed: Pharmacist-led anticoagulation management has improved prescribing appropriateness and reduced bleeding in other settings, but applying a comprehensive MTM model specifically to postoperative colorectal cancer patients is relatively novel and not yet conclusively proven.

Inclusion Criteria: 1. Patients with histologically confirmed CRC and symptomatic or incidental VTE who received anticoagulant treatment. 2. CRC patients with VTE treated with an anticoagulant for at least 3 moths. Exclusion Criteria: 1.Participation in this study required active anticoagulant treatment. Apart from this, there were no specific exclusion criteria.
