What this trial studies

This pilot project evaluates a pharmacist-led intervention aimed at addressing health-related social needs (HRSN) during transitions of care in the emergency department at Buffalo General Medical Center. The intervention includes medication reconciliation, screening for unmet social needs using the AHC HRSN tool, and connecting patients to local community resources. A total of 150 patients will be recruited, with 50 receiving the intervention and 100 serving as a control group. Follow-up assessments will measure healthcare utilization metrics and patient satisfaction with the intervention.

Who should consider this trial

Good fit: Ideal candidates are English-speaking adults aged 18 or older who are being treated at Buffalo General Medical Center and are expected to be discharged home.

Not a fit: Patients who are cognitively impaired or non-English speakers will not benefit from this study due to the intervention's communication requirements.

Eligibility criteria

Inclusion Criteria:

Age: Patients must be at least 18 years old.

Status: Patients being seen at Buffalo General Medical Center (BGMC).

Discharge Plan: It is anticipated that the patient will be discharged home. Control Group: Patients in the control group will meet the same inclusion criteria as those in the intervention group, except they will not receive the health-related social needs (HRSN) screening.

Exclusion Criteria:

Cognitive Impairment: Patients who are cognitively impaired and unable to consent will be excluded.

Non-English Speakers: Non-English speaking individuals are excluded due to the nature of the intervention requiring direct communication between the clinical pharmacist and participants
