This project will test whether a pharmacist-led care pathway in community pharmacies can help adults with type 2 diabetes and HbA1c above 7.0% lower their blood sugar and improve care compared with usual physician or nurse practitioner care.

What this trial studies

Adults with type 2 diabetes whose HbA1c is not at target will be enrolled and receive a structured pharmacist-led diabetes care pathway in community pharmacies. Pharmacists will provide medication review, shared decision-making, education, monitoring and follow-up visits, and coordinate with patients' primary care providers. Outcomes will include changes in HbA1c and management of cardiovascular risk factors such as blood pressure and lipids, measured over scheduled follow-ups. Results will be compared to usual care to see if the pharmacist-led approach improves glucose control and care processes.

Who should consider this trial

Good fit: Adults aged 18 or older with type 2 diabetes whose HbA1c is above 7.0% and who can consent and attend follow-up visits are the intended participants.

Not a fit: People with type 1 or gestational diabetes, pregnant individuals, those already at HbA1c target under 7.0%, those with limited life expectancy or frailty, or those unable to consent or attend follow-ups are unlikely to benefit from participating.

Why it matters

Potential benefit: If successful, this could make it easier for people with type 2 diabetes to reach target blood sugar levels and lower their risk of diabetes-related complications.

How similar studies have performed: Previous pharmacist-led interventions have shown modest to meaningful improvements in HbA1c and medication management, so this approach builds on existing evidence.

Eligibility criteria

Inclusion Criteria:

1. Individuals aged 18 years or older.
2. Individuals with type 2 diabetes.
3. Individuals with type 2 diabetes not reaching HbA1c target of under 7.0%

Exclusion Criteria:

1. Individuals with type 1 diabetes, gestational diabetes, or other forms of diabetes that are not type 2 diabetes.
2. Pregnant individuals.
3. Individuals at their HbA1c target (HbA1c under 7.0%) or those with a limited life expectancy, frailty, or lack hypoglycemic awareness (i.e., those with an A1c target above 7.0%) .
4. Individuals unable to provide consent or who are unwilling to attend follow-up visits.
