What this trial studies
This prospective, community-based pilot enrolls about 200 adults with uncontrolled hypertension identified through community organizations on Chicago's West and South Sides. A mobile clinical team—community health workers, a pharmacist, and a registered nurse—provides blood pressure screening, medication management, education, and care coordination over three months. The study measures participant engagement and acceptability, changes in systolic blood pressure, and the economic impact of the intervention. Results will inform whether this model can be scaled to improve hypertension control in underserved communities.
Who should consider this trial
Good fit: Adults 18 or older with uncontrolled high blood pressure (two separate recent systolic readings >130) who receive services at participating community sites on the West or South Side of Chicago and who are not pregnant, on dialysis, or post-organ transplant.
Not a fit: People with well-controlled blood pressure, those receiving dialysis, transplant recipients, pregnant people, or individuals unable to participate in community-based visits are unlikely to benefit from this pilot.

Inclusion Criteria: * Adults 18 years or older * Record of two blood pressure readings of SBP\>130 on two separate occasions (days) within the past 3 months Exclusion Criteria: * Person is receiving dialysis * Person has had a heart or kidney transplant * Person is pregnant
