What this trial studies

This project evaluates the effectiveness of a personalized prevention program (PPP) based on the Comprehensive Geriatric Assessment (CGA) for older individuals with non-communicable chronic diseases in primary care settings. The study involves 1216 participants enrolled by general practitioners across four Italian areas, with GPs randomized into intervention and control groups. The intervention group will receive tailored preventive interventions, while the control group will follow standard clinical practices. The aim is to reduce hospitalization rates and explore biological processes related to non-communicable diseases.

Who should consider this trial

Good fit: Ideal candidates are individuals aged 65 and older with at least one non-communicable chronic disease.

Not a fit: Patients under 65 years old or those without non-communicable chronic diseases may not benefit from this study.

Why it matters

Potential benefit: If successful, this program could significantly reduce hospitalization rates and improve health outcomes for older adults with chronic diseases.

How similar studies have performed: While similar approaches have shown promise in reducing hospitalization rates, this specific personalized prevention program based on CGA is novel and has not been extensively tested.
