What this trial studies
This study investigates the long-term benefits of personalized follow-up and support for patients with chronic respiratory diseases after completing a pulmonary rehabilitation program. It is a multicenter, blinded, randomized controlled trial that compares an experimental group receiving tailored support with a control group receiving standard care. Each participant will be monitored for 24 months with follow-up visits every six months, aiming to recruit a total of 352 subjects. The goal is to determine if personalized support can help maintain improvements in physical capacity and quality of life over time.

Inclusion Criteria: * Diagnosis of one or more chronic respiratory diseases (asthma, chronic obstructive pulmonary disease, diffuse interstitial lung disease, bronchiectasis, obstructive sleep apnea syndrome, etc.). * Participation in a respiratory pulmonary program * Have given consent * Patient of legal age * Patient affiliated to Social Security Exclusion Criteria: * Any medical contraindication to the practice of adapted physical activity in rehabilitation * Patients under guardianship or curatorship * Patients deprived of their liberty * Patients over 80 years of age at the time of inclusion.
