This project will try personalized care plans for people aged 60 and older who completed ICOPE Step 2 to see how many plans are produced and what they contain after one year.

What this trial studies

The program follows older adults identified through ICOPE and documents how personalized care plans (PCPs) are negotiated and implemented by primary care teams over 12 months. It uses an observational approach to record the number and content of PCPs at one year, with particular attention to negotiation methods with patients and interprofessional cooperation. Participants are recruited at the Maison de santé des Fontaines in Toulouse and must have completed ICOPE Step 2. The goal is to determine feasibility and describe typical elements of PCPs carried out in routine care.

Who should consider this trial

Good fit: People aged 60 or older with frailty who have completed ICOPE Step 2, have ADL ≥ 5.5, are covered by a social security scheme, and agree to data collection are eligible.

Not a fit: Those who have not completed ICOPE Step 2, have marked disability (ADL < 5.5), live too far from the Toulouse site, or refuse data collection are unlikely to benefit from this implementation effort.

Why it matters

Potential benefit: If successful, clearer and more tailored care plans could improve coordination of services and help preserve function in older adults with frailty.

How similar studies have performed: Previous pilot implementations of WHO's ICOPE and individualized care plans have shown promising engagement and functional outcomes in some settings, but negotiation techniques and team-based implementation strategies are still not well tested.

Inclusion Criteria:

* Age ≥ 60 years
* ADL ≥ 5.5
* Affiliated or beneficiary of a social security scheme or equivalent.
* having completed a STEP 2 ICOPE
* not opposed to the collection of their data
