This project will try a statewide opioid-use-disorder quality measurement and performance coaching approach to see if it helps people in outpatient addiction treatment, their families, and clinic staff across New York.

What this trial studies

The project implements the OASAS OUD-QM2 strategy across New York outpatient clinics using a concurrent mixed-methods design that combines administrative data, clinic surveys, interviews, and focus groups. A stepped-wedge randomized rollout will deliver performance coaching to 35 clinics in staggered sequences between years 2 and 4 while the remainder continue usual care, allowing comparison over time. Surveys of all ~550 outpatient clinics occur in years 1, 3, and 5, and qualitative interviews with patients, families, and staff are embedded to capture experiences and inform interpretation. Outcomes include retention in care and adverse events (emergency visits, hospitalizations, mortality) analyzed using statewide administrative data.

Who should consider this trial

Good fit: Adults (18+) receiving outpatient substance use treatment for opioid use disorder in New York State between 2020 and 2027, along with their clinicians and family members, are the intended participants.

Not a fit: People under 18, those treated outside New York State, patients only in inpatient settings, or those not connected to participating outpatient clinics are unlikely to receive direct benefit from this project.

Why it matters

Potential benefit: If successful, this approach could increase retention in outpatient OUD treatment and reduce emergency visits, hospitalizations, and deaths by improving clinic practices.

How similar studies have performed: Related quality-measurement and performance-coaching interventions have shown promise in other healthcare areas, but there is limited direct testing of this exact approach across a statewide OUD clinic network.

Inclusion Criteria:

* Individuals 18 years of age, and older
* Clients and clinicians of people attending substance use treatment programs in New York State with Opioid Use Disorder in the time interval 2020-2027

Exclusion Criteria:

* There are no exclusion criteria related to sex/gender to increase the generalizability of the findings. Children will not be included in this study because the treatment system mostly includes adults.
