What this trial studies

This clinical trial aims to evaluate the effectiveness of peer coaching models in promoting self-management of Type 2 Diabetes Mellitus among older adults in rural Kentucky. Participants will be paired with trained peer coaches who have successfully managed their own diabetes, and will engage in regular discussions about self-care and goal setting. The study will compare different methods of pairing coaches and participants, as well as the frequency of their interactions, to determine which approach yields the best outcomes for diabetes management. Stakeholder engagement will also be incorporated to enhance understanding of the intervention's impact.

Who should consider this trial

Good fit: Ideal candidates are adults aged 55 and older, residing in rural Kentucky, with a diagnosis of unmanaged Type 2 Diabetes Mellitus.

Not a fit: Patients under 55 years of age, those not residing in rural Kentucky, or individuals with managed diabetes or cognitive impairments may not benefit from this study.

Inclusion Criteria:

* Age greater than or equal to 55 years
* Resident in Rural Kentucky
* Positive Diagnosis of Type 2 Diabetes Mellitus
* HbA1c greater than or equal to 7.5%
* Passing score on cognitive assessment administered by study personnel
* English Speaking

Exclusion Criteria:

* Age less than 55 years
* Not a resident of a Rural Kentucky County
* No Type 2 Diabetes Mellitus Diagnosis
* HbA1c under 7.5%
* Cognitive Impairment
* Non-english speaking
