This program tries an outreach team of clinicians and municipal housing advisors to find adults with severe hoarding who are in contact with Copenhagen's Housing Advisors and connect them to regional mental health services.

What this trial studies
The project screens 60 adults with DSM-5 hoarding disorder who are currently engaged with The Housing Advisors in Copenhagen to measure rates of severe mental illness and other comorbidities. An assertive, cross-sectoral outreach team of clinicians and municipal social workers performs systematic clinical assessments in participants' homes or community settings. After informed consent, the team facilitates referrals—primarily to Flexible Assertive Community Treatment (FACT)—to regional mental health services when clinically appropriate. Participants with recent mental health contact or known organic brain disorders are excluded, and participation does not affect existing municipal supports.
Who should consider this trial
Good fit: Adults (18+) who meet DSM-5 criteria for hoarding disorder and currently have ongoing contact with The Housing Advisors in Copenhagen are ideal candidates.
Not a fit: People who have had contact with mental health services within the past six months or who have known organic brain disorders (including dementia or intellectual disability) may not benefit from this outreach intervention.
Why it matters
Potential benefit: If successful, the program could greatly increase access to appropriate psychiatric care for people with severe hoarding, reducing safety risks, social marginalization, and eviction threats.
How similar studies have performed: Prior research has mostly studied help-seeking groups, so cross-sectoral outreach for municipal hoarding cases is relatively novel, though assertive community approaches have shown benefit in other severe psychiatric populations.

Inclusion Criteria: * Presence of hoarding disorder according to DSM-5 and age above 18 years. * Current contact with The Housing Advisors, Copenhagen Municipality. Exclusion Criteria: * Recent contact with mental health services (within last 6 months). * Previously known organic brain disorder (including mental retardation and dementia).
