What this trial studies

This study investigates the impact of a clinical medication review combined with a deprescribing toolbox on reducing the number of medications in frail older adults who use multidose drug dispensing systems. Conducted in 38 community pharmacies in the Netherlands, the trial will involve pharmacists trained to perform a structured 5-step review process with patients aged 75 and older who are on more than 10 medications. The intervention aims to enhance patient-centered care by incorporating patient preferences and follow-up evaluations to ensure effective deprescribing. The study will compare outcomes between patients receiving the intervention and those receiving usual care.

Not a fit: Patients who have received a clinical medication review in the past 12 months or those living in nursing homes may not benefit from this study.

Eligibility criteria

Inclusion Criteria:

* using a multidose drug dispensing system
* hyperpolypharmacy patient (using 10 drugs or more)
* age: 75 years or older

Exclusion Criteria:

* received a CMR in the previous 12 months
* incapacitated
* patients for whom the general practitioner is not the primary practionar
* expected lifespan of less than six motnhs
* patients who live in a nursing home
