This will test whether an eight-module online Acceptance and Commitment Therapy (ACT) program can help adults with misophonia feel better compared with being on a waitlist.

What this trial studies
This randomized controlled trial will enroll 100 adults who meet clinical criteria for misophonia and randomly assign them to either an automated eight-module online ACT program or a delayed-treatment (waitlist) control. Intervention participants complete the modules and five surveys over four months while waitlist participants complete surveys and receive access after study completion. Primary outcomes include misophonia symptoms, with secondary outcomes of quality of life, general distress, and psychological flexibility measured at baseline, mid-intervention, post-intervention, and one- and two-month follow-ups. Multilevel modeling will test time-by-condition interactions to determine whether the online ACT program both reduces symptoms and is acceptable to users.
Who should consider this trial
Good fit: Adults aged 18 or older who live in the United States, speak English, and currently meet clinical impairment criteria for misophonia without concurrent psychotherapy for misophonia or recent changes to psychotropic medication are the ideal candidates.
Not a fit: People with active self-harm or psychosis, those currently in psychotherapy for misophonia, those who have recently started or changed psychotropic medication, or those unable/unwilling to use an online program may not benefit or be eligible.

Potential benefit: If successful, this program could provide an accessible, scalable online treatment that reduces misophonia symptoms and improves daily functioning and quality of life.
How similar studies have performed: Prior studies have shown promise for ACT in treating misophonia symptoms, but fully automated online ACT interventions for this condition are still relatively untested.

Inclusion Criteria: * Currently meet clinical impairment for misophonia. * At least 18 years old. * Fluent English speakers. * Currently live or reside in the United States Exclusion Criteria: * Currently receiving alternative psychotherapy for misophonia. * Currently modifying or starting psychotropic medication (within 30 days of starting the study). * Any psychological and/or neurological impairments that would preclude someone from participating in the study (e.g.,active self-harm or psychosis)
