This project will test whether a government mHealth app that delivers on-the-spot supportive messages combined with occasional nurse consultations helps older adults with diabetes manage their condition better.

What this trial studies
Participants who have had diabetes for at least one year and use the government mHealth app will receive just-in-time adaptive interventions delivered through the app plus proactive nurse consultations, while a comparison group will receive an active control condition. The study measures changes in self-management behaviors and app usage, using surveys and digital usage logs to capture outcomes. People already enrolled in other mHealth programs or with significant physical or cognitive limitations are excluded. The intervention is delivered through the District Health Center in Hong Kong and requires internet access for real-time support.
Who should consider this trial
Good fit: Ideal candidates are older adults in Hong Kong with a confirmed diabetes diagnosis for one year or more who already use the government mHealth app and live in an area with internet coverage.
Not a fit: Patients with major physical or cognitive limitations, those without reliable internet access, or those already enrolled in other mHealth programs may not benefit from this intervention.

Inclusion Criteria: * With a confirmed DM diagnosis for one year or more than one year * Using governmental mhealth App * Living in an area with Internet coverage Exclusion Criteria: * Already engaged in other mHealth programmes * Physical and cognitive limitations
