What this trial studies
This study aims to implement and evaluate a 4-month nutrition-enhanced intervention led by community health workers (CHWs) for primary care patients suffering from nutrition insecurity and poorly controlled hypertension. The intervention builds upon an existing CHW program that provides education on hypertension management, medication adherence, and blood pressure self-monitoring. Participants will receive tailored nutrition education materials focusing on healthy eating habits, including reducing sodium and increasing fruits and vegetables. The program is designed to address the specific needs of food-insecure populations with limited resources.
Who should consider this trial
Good fit: Ideal candidates are adults aged 21 and older with uncontrolled hypertension who are either low-income or identify as a minoritized race/ethnicity.
Not a fit: Patients who do not speak English or Spanish or who decline participation in the CHW hypertension program may not benefit from this study.

Inclusion Criteria: * Eligible to participate in the Massachusetts General Hospital CHW hypertension program (age 21 years or older, uncontrolled hypertension, patient at one of 5 participating primary care clinics; either identifies as a minoritized race/ethnicty or is low-income); speaks English or Spanish Exclusion Criteria: * declines participation in CHW hypertension program; does not speak English or Spanish
