This study is testing a nutrition and education program for adults with diabetes who use food pantries to see if it helps them manage their condition better while also addressing their food needs.

What this trial studies
This project aims to provide diabetes self-management education and support (DSMES) while addressing food insecurity among high-risk, ethnically diverse adults participating in a food pantry. Utilizing a Community-Based Participatory Research model, community representatives will help design culturally impactful messaging and advise on study operations. Participants will undergo diabetes screening and receive tailored education through web-based modules, peer support, and monthly group sessions facilitated by nursing students and registered dietitian nutritionists. Data will be collected at enrollment and again 90 days later to assess the program's effectiveness.
Who should consider this trial
Good fit: Ideal candidates are adults aged 18 and older with a diagnosis of Type 2 Diabetes Mellitus who are participants in a food pantry.
Not a fit: Patients who do not have a diagnosis of Type 2 Diabetes Mellitus or are under 18 years old will not benefit from this study.

Inclusion Criteria: * Age 18 and older * Existing and/or new diagnosis of T2DM * Food pantry participants * English and/or Spanish speaking * Willing/able to provide informed consent * Must have a mobile device able to receive text messages Exclusion Criteria: * Under the Age of 18 * No T2DM or do not meet HbA1c \> 6.5 criteria * Not a food pantry participant * Unwilling/unable to provide informed consent * Spoken language is other than English/Spanish * Individuals who do not have access to a mobile device or are unable to receive text messages
