What this trial studies
This is a prospective, open-label 1:1 randomized controlled trial conducted at the National Institute of Neuroscience & Hospital (Dhaka) from January to September 2026 with a planned sample of 166 participants. The intervention is a 6-month nurse-led rehabilitation program delivered with a multidisciplinary team, focusing on self-care training and use of assistive devices and involving family caregivers. Eligible participants are adults discharged from NINS&H within two weeks after stroke with moderate disability (mRS 2–4) who can communicate and consent. Outcomes will compare functional ability and community participation between the nurse-led program and usual care.
Who should consider this trial
Good fit: Adults (18+) recently discharged from NINS&H within two weeks after stroke with moderate disability (mRS 2–4), who need assistive devices, can communicate, have a phone, and can give consent are ideal candidates.
Not a fit: Patients with very severe disability (mRS >4), those who cannot communicate, those already receiving institutional rehabilitation, or those enrolled in other trials are unlikely to benefit from this program.
Why it matters
Potential benefit: If successful, the program could help stroke survivors regain independence in daily activities and increase their participation in community life.
How similar studies have performed: Multidisciplinary stroke rehabilitation has shown improved functional outcomes in many settings, although nurse-led, community-focused programs in Bangladesh are less well studied.

Inclusion Criteria: 1. 18 years old and above, both males \& females, 2. Irrespective of the type of stroke and time of stroke, 3. mRS 2-4 with disability on upper and/or lower limb(s) 4. Physician advice on rehabilitation, and who needs assistive devices for ADL, 5. Having a phone, and 6. Willing to provide consent \& participate in the study. Exclusion Criteria: 1. Participation in other clinical trials, 2. Planning to receive institutional rehabilitation services, and 3. Cannot communicate (having speech difficulties).
