What this trial studies
This pilot interventional study aims to evaluate the feasibility and acceptability of a supportive intervention for patients with heart failure. Participants will receive a combination of pre-discharge education, nurse-led telephone coaching, and home telemonitoring of vital signs. The study will assess the effectiveness of these implementation strategies in enhancing patient adherence and satisfaction, as well as the completeness of data collection. Ultimately, the goal is to compare outcomes between the intervention group and a control group to determine the impact on hospitalization rates and self-care capabilities.
Who should consider this trial
Good fit: Ideal candidates are individuals aged 65 and over who have been hospitalized for cardiac decompensation and are expected to be discharged home.
Not a fit: Patients who lack the cognitive or physical ability to self-monitor their vital signs or do not have a caregiver to assist them may not benefit from this study.

Inclusion Criteria: * aged 65 and over * hospitalized for cardiac decompensation, regardless of ejection fraction (FE) value (preserved or decreased) * expected to be discharged home will be considered eligible. Exclusion Criteria: * Individuals who lack the cognitive and/or physical capabilities (Mini-COG +) for self-monitoring of vital signs, and without a caregiver available to assist them * people who receive other medical services
