What this trial studies

This project evaluates innovative, sustainable models for managing hypertension in primary health care settings. It involves a pilot study at two Primary Health Care Centres where cardiovascular risk is assessed using SCORE2 for all hypertension patients. Staff will receive training on hypertension treatment protocols, and non-physicians will lead group education sessions for patients. Patients will use home blood pressure monitors and communicate digitally with nurses for medication updates and follow-ups, while physicians will only meet with high-risk patients as necessary.

Who should consider this trial

Good fit: Ideal candidates are adult patients diagnosed with hypertension.

Not a fit: Patients with secondary hypertension, terminal illness, or cognitive impairments may not benefit from this study.

Why it matters

Potential benefit: If successful, this approach could lead to improved health outcomes and cost-efficiency in managing hypertension.
