What this trial studies
This project evaluates innovative, sustainable models for managing hypertension in primary health care settings. It involves a pilot study at two Primary Health Care Centres where cardiovascular risk is assessed using SCORE2 for all hypertension patients. Staff will receive training on hypertension treatment protocols, and non-physicians will lead group education sessions for patients. Patients will use home blood pressure monitors and communicate digitally with nurses for medication updates and follow-ups, while physicians will only meet with high-risk patients as necessary.

Inclusion Criteria: \- Adult patients with hypertension Exclusion Criteria: * secondary hypertension according to the medical records at the Primary Health Care Center * terminal illness * pregnancy-induced hypertension * cognitive impairment * impaired vision (not able to read measurements for the home BP measurements or communicate via "1177 direct" digitally) * psychotic disorder
