This program will try a multi‑mode form of Cognitive Processing Therapy to reduce PTSD symptoms and improve sleep and mood in adults aged 20–65 with PTSD.

What this trial studies

This is a single‑blind, randomized, two‑arm longitudinal trial comparing Multi‑Mode Cognitive Processing Therapy (MMCPT) to treatment‑as‑usual (TAU) for patients with DSM‑5 PTSD. MMCPT follows the Resick CPT manual delivered as twelve weekly 90‑minute individual sessions by trained therapists, with rigorous monitoring of treatment fidelity and safety. Primary outcomes are clinician‑rated and self‑report PTSD severity (CAPS‑5 and PCL‑5); secondary outcomes include sleep measures (actigraphy, PSQI), anxiety (STAI), depression (HDRS‑17), remission rates, and posttraumatic growth (PTGI). Participants are assessed at baseline, mid‑treatment, post‑treatment, and at 3, 6, 9, and 12 months to capture short‑ and longer‑term effects.

Who should consider this trial

Good fit: Adults aged 20–65 with a DSM‑5 diagnosis of PTSD, a PCL‑5 score of 33 or higher, ability to consent, and a stable psychiatric medication regimen (if any) for at least 4 weeks are the intended participants.

Not a fit: People with psychotic disorders, bipolar I, current substance dependence, recent severe suicidal behavior, significant cognitive or neurological impairment, or those already in other psychological PTSD treatments are unlikely to benefit from participating.

Eligibility criteria

Inclusion Criteria:

* Aged between 20 and 65 years
* Diagnosed with PTSD according to DSM-5 criteria
* Score ≥ 33 on the PTSD Checklist for DSM-5 (PCL-5)
* Able to provide informed consent
* Stable psychiatric medication regimen (if any) for at least 4 weeks prior to enrollment

Exclusion Criteria:

* Diagnosed with schizophrenia, schizoaffective disorder, or bipolar I disorder
* Current substance dependence or abuse within the past 6 months
* Severe suicidal ideation or suicide attempt in the past 6 months
* Cognitive impairment or neurological disorder affecting participation
* Concurrent participation in other psychological treatment for PTSD
