This project will test whether a multimedia video education program helps adults with mild-to-moderate chronic kidney disease increase physical activity and improve fitness over 12 weeks compared with standard education.

What this trial studies

Adults with CKD stage 1–3b will be randomly assigned to a multimedia education group or a standard education group and followed for 12 weeks. The multimedia group will receive video-based health education delivered via mobile devices while the control group receives conventional patient education. Primary outcomes include cardiorespiratory endurance, muscle strength, flexibility, and physical activity levels measured before and after the intervention. The trial will also monitor changes in health behaviors and adherence to exercise recommendations.

Who should consider this trial

Good fit: Ideal candidates are literate adults aged 20 or older with CKD stages 1–3b who can understand Mandarin or Taiwanese, operate a mobile device (with caregiver help if needed), and are medically cleared to exercise.

Not a fit: Patients with advanced CKD, uncontrolled cardiovascular disease, significant gait impairment or limb amputation, or cognitive/psychiatric conditions that prevent following the program are unlikely to benefit.

Inclusion Criteria:

* Conscious adults aged 20 years or older who are literate and able to communicate in Mandarin or Taiwanese.
* Diagnosed with chronic kidney disease (CKD) stage 1 to 3b according to the CKD-EPI formula and confirmed by a nephrologist.
* Able to operate a mobile device (such as a smartphone, tablet, or computer) and have access to equipment capable of playing multimedia videos; family members or caregivers may assist if necessary.
* Able to view and understand multimedia educational video content.

Exclusion Criteria:

* Individuals with cognitive or psychiatric disorders that prevent understanding or adherence to the exercise program.
* Individuals with gait instability or limb amputation who are unable to perform the exercise protocol.
* Individuals with poorly controlled cardiovascular disease, such as heart failure, myocardial infarction, angina, arrhythmia, or those who have experienced chest pain during activity or rest within the past three months.
* Individuals deemed unsuitable for exercise by their attending physician.
* Individuals with poorly controlled diabetes (HbA1C \> 9%) accompanied by retinopathy or foot ulcers.
