What this trial studies
This observational study aims to evaluate the feasibility of regular multidisciplinary team conferences involving various medical specialists to discuss the care of patients with diabetes and concurrent chronic conditions. The study will assess how these meetings can be integrated into everyday hospital practice and what benefits they provide to both patients and doctors. Patients will be invited to participate by having their cases discussed and will complete surveys regarding their well-being before and six months after the meetings. Doctors will also provide feedback on their learning experiences from these conferences.
Who should consider this trial
Good fit: Ideal candidates for this study are adults with any type of diabetes who are managing their condition with a general practitioner or at the Steno Diabetes Centre Odense and have at least one additional chronic condition related to heart, kidney, or lung health.
Not a fit: Patients without diabetes or those who do not have any additional chronic conditions may not benefit from this study.

Inclusion Criteria: * Adult, any type of diabetes, diabetes management by general practitioner or Steno Diabetes Centre Odense, minimum one chronic condition within heart, kidney or lung specialities. Exclusion Criteria: * none
