What this trial studies

This implementation study aims to enhance aftercare for patients with acquired brain injury (ABI) who are undergoing outpatient rehabilitation. The ABI-motion program focuses on promoting an active lifestyle and preventing ongoing complaints and poor health-related quality of life (HR-QoL) post-rehabilitation. Participants will receive brain education, joint therapy sessions with therapists and support buddies, and follow-up care from rehabilitation physicians. The study will assess the feasibility and health benefits of this comprehensive aftercare approach.

Who should consider this trial

Good fit: Ideal candidates are individuals diagnosed with acquired brain injury who are currently participating in outpatient rehabilitation.

Not a fit: Patients who have contraindications for moderate to vigorous exercise or a life expectancy of less than one year may not benefit from this program.

Inclusion Criteria:

* Diagnosed with ABI
* Follows an outpatient rehabilitation program for ABI in Rijndam Rehabilitation

Exclusion Criteria:

* Having a contra-indication for participation in moderate to vigorous exercise
* Life expectancy \< 1 year
* Incapacitated persons
