What this trial studies
This study evaluates the effectiveness of a mobile application designed to support patients recovering at home after elective colorectal surgery. It aims to reduce unplanned hospital visits by providing an integrated discharge monitoring system. Patients will be randomly assigned to either a control group receiving standard follow-up care or an intervention group using the mobile app. The study will take place across three high-volume colorectal surgery centers in Canada over three years.
Who should consider this trial
Good fit: Ideal candidates are adults aged 18 and older who are being discharged home after elective colorectal surgery.
Not a fit: Patients who are discharged to rehabilitation centers or have post-operative admissions of less than three days may not benefit from this study.

Inclusion Criteria: * any patient who is being discharged to home after undergoing elective colorectal surgery * 18 years or older * able to speak and understand English or French * provides informed consent Exclusion Criteria: * post-operative admission less than 3 days * requiring discharge to rehabilitation centre to recover prior to being able to go home
